New Patient Visit
Your 1st Visit
Welcome to our clinic! We look forward to providing you with natural healing at the highest quality. You may schedule an appointment using our online scheduling system or call us at (650) 924-9098. Please fill out and sign all the forms prior to your first appointment.
After you've made your first appointment, you'll receive a welcome letter email along with the new patient forms from our office. You may email them to firstname.lastname@example.org or bring them with you to your appointment. Your first acupuncture appointment will last for approximately 60-90 minutes. Please make sure you are not too hungry prior to treatment.
For telehealth patients, please also fill out the telehealth consent form prior to appointment.
If you would like to have your insurance cover your treatment, you may call your insurance to verify your acupuncture benefits.
You will need your Member/Subscriber ID, name, and date of birth in order to have them access your account. The followings are
the questions you ask to verify your acupuncture benefits:
- What is my deductible and does it apply to acupuncture services?
- Have I met my deductible?
- What is my copay/coinsurance for aucpuncture?
- How many acupuncture visits do I have per year? How many have I used?
Emily is "in network" with the following insurances:
- Cigna PPO
- United Healthcare
- Worker's Comp and PI cases are accepted
For Out-of-Network insurances, we'll provide you a superbill upon request.
Strict 24-hour Cancellation Policy
If you need to cancel or reschedule please kindly give us 24 hours notice in advance. Last minute cancellations will result in full fee charge. The cancellation fee is to enhance fairness for other patients in need to get treatment during that time slot.
